As soon as are in the Ward Prime area, head straight to the main computer area towards the Southeast and find the Root Nexus. Interact with it to enter the Root World. Now, head Northeast and destroy a Root Heart out there. This will pave the way to the Medical Bay area.
The Prime Maintenance Key in Remnant: From The Ashes is hidden in this Medical Bay area. To get the key, you will first have to repeat the process of destroying a Root Heart in the Bay. Once you are done with that find a window. You will find a wooden bookshelf when you jump through the window. Behind this bookshelf is the hidden area where you will find the Prime Maintenance Key.
Destroy the bookshelf and enter the hidden area, the Prime Key will be awaiting you. Grab the Key and head back to the location where you destroyed the first Root Nexus. You will find a small locked door on the left of the double doors. Unlock it with the Prime Maintenance Key and the Key will turn green from red.
